The aim of this review is to provide an overview of the latest data available about the pharmacokinetics and pharmacodynamics of sildenafil citrate [a] and sildenafil-containing therapies, such as sildenafil (Viagra), tadalafil (Cialis), vardenafil (Levitra), and avanafil (Stendra).
This review will cover information from three pivotal studies: (1) a randomized, double-blind, placebo-controlled crossover study, (2) a randomized, three-way crossover study, and (3) a Phase III study in patients with erectile dysfunction. The first two studies were conducted in South Africa and the third study, in the United States, was performed in the United Kingdom. Both of the studies examined the pharmacokinetics and safety of sildenafil citrate [] and sildenafil-containing therapies, and both studies showed significant improvements in erectile function after sildenafil administration [b] versus placebo in patients with mild to moderate erectile dysfunction. In addition, these studies showed that sildenafil was well tolerated and that the safety profile was comparable between sildenafil and tadalafil.
The purpose of this review is to provide an overview of the latest data available about the pharmacokinetics and pharmacodynamics of sildenafil and tadalafil in South Africa, the United Kingdom, and the United States, as well as to provide information about the safety of these therapies and to discuss the clinical and regulatory requirements for these drugs.
In the South African and United Kingdom Trials (SB-UK and UK-UK), sildenafil was well tolerated and safe (; ), but efficacy was reported to be modest. In the United Kingdom Trials (UK-UK), tadalafil was well tolerated and safe (; ), but efficacy was reported to be modest. In the United States Trials (US-UK), tadalafil was well tolerated and safe (; ; ; ; ). Both studies reported adverse events similar to those reported in the UK Trials (UK-UK) and US-UK (US-UK), including headache, flushing, upset stomach, and abdominal pain. However, the adverse events in the UK Trials (UK-UK) were more severe and included severe headache, urinary retention, hypotension, syncope, and priapism (; ; ).
The results of the three studies have been summarized inTable 1.
The efficacy and safety of sildenafil were demonstrated in two of the studies. The studies showed that sildenafil was well tolerated and safe (; ), with an incidence of ≥1% in the tadalafil group and ≥2% in the sildenafil-containing group. Safety data were available for only one study in the United States, which reported on a total of 829 adverse events, including ≥1% in the tadalafil group and ≥1% in the sildenafil-containing group. A significant increase in the mean t1/2 was observed in the tadalafil-containing group compared with the sildenafil-containing group at 12 weeks, while no significant changes were observed in the tadalafil- and sildenafil-containing groups. Although the incidence of the adverse events in the US trials was <5% in the tadalafil group compared with the sildenafil-containing group, it was approximately 1.5% in the tadalafil-containing group. The adverse events that occurred in the tadalafil- and sildenafil-containing groups were headache, flushing, nausea, vomiting, diarrhea, headache, and priapism. There were no statistically significant differences in the adverse events between the groups.
In the UK Trials, tadalafil was well tolerated and safe (; ; ; ; ). In the US Trials, tadalafil was well tolerated and safe (; ; ; ). There were no statistically significant differences in the adverse events that occurred in the tadalafil- and sildenafil-containing groups. Safety data were available for only one study in the United States, which reported on a total of 531 adverse events, including ≥2% in the tadalafil-containing group and ≥1% in the sildenafil-containing group.
